Nomination Form:

OMMC encourages excellence and celebrates individual achievements as well as outstanding projects within the military museum community.

The OMMC Honours and Awards program responds to this statement and is managed by the Honours and Awards Committee as designated by the OMMC Board of Directors. This form can be used to nominate an OMMC member for one of the following categories:

1. OMMC Award of Excellence
2. OMMC Award for Projects

The deadline for nominations: 31 May 2026

Only OMMC members in good standing at the time of nomination are eligible. This form may be used for one nomination only. Please complete another form if you wish to make more than one nomination.

The Honours and Awards Committee is committed to respecting everyone's privacy. All nominations are confidential and personal information therein included will not be disclosed to third parties.
